"Another disease investigated in this report is Type 1 diabetes (TID), an autoimmune, polygenic disease. Of the several T1D loci, IDDM8 on 6q, has been found to be subject to parent-of-origin effects and encompasses IGF2R. M6P/IGF2R is involved in immune system regulation. In this study we show an association between TID and IGF2R that is confined to maternally inherited alleles. Our results strongly suggest that IGF2R is a TID susceptibility gene and may be universally imprinted at some tissue or developmental stage not yet studied."--

Effective recruitment and retention strategies in community health programs(

Abstract : Issue addressed: The aim of this project was to identify effective recruitment and retention strategies used by health-promotion organisations that focus on increasing physical activity and improving nutrition within the local community. Methods: Semistructured telephone or face-to-face interviews with 25 key informants from stakeholder organisations were conducted. Key informants discussed strategies used by their organisation to effectively recruit and retain participants into community-based healthy eating and/or physical activity programs. Transcribed data were analysed with NVivo software. Results: Effective recruitment strategies included word of mouth, links with organisations, dissemination of printed materials, media, referrals, cross-promotion of programs and face-to-face methods. Effective retention strategies included encouraging a sense of community ownership, social opportunities, recruiting a suitable leader and offering flexibility and support. Fees and support for recruiting and retaining participants was also identified. Conclusion: This study provides novel insights to a greatly under researched topic in the field of health promotion. There are two key take-home messages from the present study that are applicable to health practitioners as well as developers and deliverers of community health-promotion programs: (1) it is imperative that all community health organisations report on the effectiveness of their recruitment and retention, both successes and failures; and (2) there is a clear need to tailor the recruitment and retention approach to the target population and the setting the program is occurring in. So what?: These findings provide important insights for the development of future community-based healthy eating and physical activity programs. A particular criticism of this "mini-budget" process has been the lack of detailed information to enable substantive input from the Assembly and the wider public on the proposed reprioritisation of spending allocations between departments. The full implications for Northern Ireland of the current deficit in UK public sector finances still have to emerge but the coming years will be a period of exceptional budgetary constraint for the local public sector. Pressure is already mounting to maintain or even increase service delivery with decreasing resources. This presents a challenge for the Executive for the future given the scale and immediacy of additional budgetary pressures and that the Northern Ireland Departments have already been working to achieve cumulative efficiencies of between 2 to 3 percent over each of the last six years. The Committee's aim in this inquiry was to examine how the Executive can maximise efficiencies without having an adverse impact on essential front line services and strategic policy priorities. It also sought to clarify around the nature of proper efficiency gains and how they differ from straightforward budget cuts. Also examined was how to monitor and oversee the delivery of efficiencies. Expert witnesses with experience of efficiency programmes locally and in other jurisdictions were examined and a wide range of other sources were drawn from and a literature review of best practice in this subject area. Some key themes have emerged, the foremost of these is for the Executive to set out clearly the essential services and strategic policies which are to receive priority, in terms of ensuring the level and quality of services are not lessened as a result of the planned savings
