INTENSIVE OUTPATIENT DIABETES CARE THROUGH A FULL SHARING CLINICAL INFORMATION SYSTEM by Vincenzo Provenzano( )

Background: In a previously experience the adoption of a diabetes-oriented clinical supporting system, shared by diabetes centers and primary care, we found that it is possible to reach the composite end-point of HbA1c <7%, LDL < 100 mg/dl, Blood pressure (SBP) < 140 mmHg in a higher percentage of type 2 diabetic patients (T2 DM) than with normal care (from 2012 to 2015 37,2 % and 51.8 % at target respectively for a population of 42498 subjects). Methods: we studied 641 T2 DM (44,8 % women), with diabetes control not at target or unstable, through Smart Digital Clinic (previously MYSTAR) consecutively attending our integrated outpatient cljnic (IOC) , where every visit was performed in conjunction with family doctor. Clinical alerts were used for good control and missed visits. All of them underwent lab test, DM complications screening and received educational support. End-point of analysis were good diabetes control (Hba1c < 7%), SBP at target (prev <140 mmHg now < 130 mmHg) and LDL < 100 mg/dl. Results: composite end-point, from 2015 to 2018. was significantly reached in a higher percentage of patients attending IOC (2015 value 51,6 % vs 2018 value 58,3 % p< 0.05). SBP < 130/80 mmHg percentage was 52,3%, LDL < 100 mg/dl was 55% from 2015 to 2018 (2012-2015 57,1 % ) but LDL < 70 mg/dl was 13,6%, HbA1c < 7% was actually reached in 87.2% of subjects. If we consider 1 visit/year for those only on diet, the achievement of the above results was with mean visit of 1,2 if on insulin and 1,3 on insulin plus oral antiabetic drug. Conclusions: the use of clinical supporting systems, diabetes care-oriented, is a valid tool of surveillance on our clinical strategies aiming to a good diabetes control and cardiovascular risk-factors. Alerts and cut-off analyzing algorithms support and control our performance in a significant way
