The hemodynamic evaluation of patients with acute circulatory failure and respiratory failure has in the past usually been performed using invasive procedures but in recent years less invasive monitoring devices have been introduced. Hemodynamic evaluation by echocardiography is based on the integration of simple indices that can be easily acquired within a few minutes at the bedside. Echocardiography can be used for both the diagnosis and the management of circulatory and respiratory failure. This book provides all the essential information required by readers in order to perform optimal hemodynamic management of the critically ill based on echocardiographic guidance. After an introductory section on basic principles, hemodynamic assessment using echocardiography is discussed in detail. The diagnosis and management of all types of circulatory and acute respiratory failure by means of echocardiography are then rigorously considered, and specific situations such as thoracic trauma and acute aortic syndrome are examined. The final section is devoted to future issues and applications

Contexte : La chirurgie de l'anévrisme de l'aorte abdominale par clampage sus-rénale de l'aorte (CSRA) est pourvoyeuse d'insuffisance rénale. L'ischémie-reperfusion (I-R) induite par cette procédure entrainerait une dysfonction endothéliale avec une baisse de la tétrahydrobioptérine (BH4), cofacteur des NO synthases. L'objectif de l'étude est de montrer que l'I-R par CSRA induit une altération de la microcirculation rénale que le BH4 préviendrait. Méthodes : Dix neuf brebis adultes ont été anesthésiées, ventilées mécaniquement et monitorées de manière invasive. Aprés une lombotomie gauche, le débit sangin rénal était mesuré en continu à l'aide d'une sonde de débit placée autour de l'artére rénale et un clamp était positionné au niveau de l'aorte susrénale. Les animaux ont été randomisés en 3 groupes (contrôle=5, I-R=7, I-R+BH4=7). Le groupe contrôle a été opéré sans CSRA. Les groupes I-R ont été exposés à une heure de CSRA. Le groupe BH4+I-R a bénéficié d'un prétraitement par 20mg/kg de BH4. Tous les sujets ont été surveillés jusqu'à 6 heures après la reperfusion. La microcirculation rénale a été évaluée par la vidéomicroscopie Sidestream Dark-Field à l'état basal à 1, 4 et 6 heures après la reperfusion. Nous avons calculé la densité des petits vaisseaux perfusés (PVD) comme le nombre de petits vaisseaux perfusés/mm (v/mm), la proportion des petits vaisseaux perfusés (PPV) et l'hétérogénéïté de PPV (PPV-HI). Résultats : Les variables hémodynamiques étaient conservées dans les 3 groupes. Le BH4 a amélioré la PVD (3.0 (2.6-3.3), 2.5 (2.0-3.1) 3.1 (2.4-3.8) v/mm) comparé au groupe I-R (2.0 (1.5-2.8) 2.1 (1.7-2.7) 1.6 (1.3- 2.3) v/mm) respectivement à 1, 4, et 6 heures de la reperfusion (P<0.05). A 1 heure de la reperfusion, le groupe I-R+BH4 a présenté une PPV et une PPV-HI mieux préservées par rapport au groupe I-R (PPV 92.3 (89.8- 93.8) versus 80.1 (73.5- 90.2)%, P<0.05 et PPV-HI 24.7 (17.1- 32.8) vs 43.0 (22.2- 84.0)%, P=0.056). Le BH4 a été associé à une amélioration de la fonction rénale, évaluée par la créatininémie à 6 heures de la reperfusion, dans le groupe I-R+BH4 (14 mg/L (13-17)) par rapport au groupe I-R (16 mg/L (16-22)) (P=0.072). Conclusion : Dans un modèle ovin d'I-R rénale un prétraitement par BH4 a amélioré les lésions et les dysfonctions microvasculaires. Des essais cliniques sont nécessaires pour évaluer la prévention par le BH4 de l'insuffisance rénale induite par l'ischémie-reperfusion
