Survival of children treated for brain tumors has improved during the last 25 years and, in long-term survivors, late sequelae have become a major concern. We evaluated 61 long-term survivors to determine neurological impairment, disability and handicap after treatment. Patients and methodsČ From 248 children registered at the Cancer Regioster of Slovenia, treated for brain tumor between 1969 and 1992, 99 are still alive and 4 are lost from the followup. Of 99 patients (pts) 88 were at least 15 years old and have been observed for at least 3 years after treatment. Nineteen patients refused to beevaluated, 2 are living outside Slovenia and we didnćt find medical documentation for 6 pts. The final sample therefore consisted of 61 pts (20 females, 41 males, aged 1-16 years at diagnosis). They were evaluated at the age of 15-42, 5-28 years after treatment. Nine pts had only surgery, 3 had only radiotherapy (RT), 4RT and chemotherapy (CHT), 15 surgery, RT and CHT and30 pts had surgery with RT. All 61 pts were interviewed regarding their social life, schooling and employment for evaluation of their handicap. Neurological examination included computed tomography (CT) or magnetic resonance imaging (MRI) of the brain (all pts) and ophthalmologic examination (47 pts). Psychological evaluation (54 pts) included Wechsler Bellevue Intelligence test. Disability was classified into 4 categories (none-severe) regarding pts ability of self care. The data were analysed using decision-trees method. ResultsČ Thirty-eight pts (70%) had at least one impairment. Visual impairment was detected in 14 pts (24%) and is associated with recurrence (p=0.012). Thirty-gour pts (56%) had motor impairmentč in the group of the irradiated pts sex (female) is the most important risk factor. Thirteen pts (21%) had epilepsy. It is associated with supratentorial locationof tumor (p=0.001). CT scan showed generalised brain atrophy in 13 pts(21%)č (Abstract truncated at 2000 characters.)

Neuroblastoma is a malignant tumor of the sympathetic nervous system, representing about 5 percent of all childhood malignancies. The survival of neuroblastoma patients remains poor in spite of some improvement in treatment. A retrospective clinical analysis has been performed of neuroblastoma patients, treated in two periods of time, 1970-1979 and 1980-1993. These periods were chosen because several improvements in the diagnostic methods as well as treatment have been introduced after 1980. Sonography, computerized tomography, I-31 metaoidobenzylguanidine (MIBG) isotope scanning in diagnostics and new cytotoxic agents, new chemotherapeutic protocols, treatment with I-131 MIBG and bone marrow transplantation have been introduced in therapy. The question was whether, due to these improvements, there has been any improvement in the survival of neuroblastoma patients after 1980. Seventy-eight neuroblastoma patients, treated at the Department of Pediatrics and at the Institute in Oncology in Ljubljana 1970-1993 were included in the study. The observation period was from first admission for neuroblastoma until last follow-up or until death. The follow-up time was 0.06-24.7 years, median 1.6 years. A univariate statistical analysis has been performed of the time period as well as the prognostic factors: stage, age at diagnosis, sex, primary site and tumor size, VMA and catecholamine concentration in urine at diagnosis. The factors, which in univariate statistical analysis were found to influence survival significantly, were the, together with the period of time, included into a multivariate survival analysis. A univariate analysis has also been performed of the influence of the primary treatment response and of the extent of surgery on the survival. In the multivariate analysis the period of time (relative risk (Rr)=0.43, p=0.19), the size (Rr=3.44,p=0.06) and site of primary tumor (Rr=1.3-3.5,p=0.013), the stage of disease (Rr=2.75-6.95,p=0.(trunc.)

Introduction: With evaluation of late sequelae in long term childhood cancer survivors we recognize risk factors for their occurence, can define groups of survivors with high or low risk and consequently plan treatment accordingly tominimize late sequelae. After treatment of nephroblastoma, renal function can be imaired as a consequence of nephrectomy, irradiation of the remaining kidney and/or chemotherapy. At present, nephrectomy is mandatory and in last decades there is a tendency to replace irradiation treatment with chemotherapy. While the effects of irradiation are rather well known, the lateeffects after chemotherapy alone and after combined chemotherapy and radiotherapy are not. There are only few studies of renal function in long time survivors after treatment for nephroblastoma, the results of which are not all in agreement. AimČ To evaluate late effects of nephroblastoma treatment on kidney size and function. To analyze the influence of different risk factors like the age at treatment, mode of treatment (irradiation, chemotherapy) and follow-up period on the occurence of late effects. Finally, to define the most suitable method for detection of late effects on kidney function. Patients: Twenty-two long time survivors, 2 months - 16 years old atdiagnosis, treated for nephroblastoma between 1969 - 1987 at the Pediatric clinic and/or Institute of oncology in Ljubljana were evaluated. The time of follow-up was 12,2 - 29,7 years (mean 24,4 years). At the time of treatment all had nephrectomy, 21 received chemotherapy, of which 3 received Avtinomycin(AMD) only, 13 AMD and Vincristin (VCR), 3 AMD, VCR and Adriamycin (ADR), 1 VCR, Adria, DTIC and Endoxane and 1 AMD, VCR, DTIC, Adriamycin and Endoxane. Six patients had no radiation treatment, 8 had tumor bed irradiation, 6 had whole abdominal irradiation and 2 had lung irradiation. (Abstract truncated at 2000 characters)
