Introduction: Current epidemiological data show that overweight and obesity in adolescents belong to the health problems that general practitioners deal with on a frequent basis. However, there are very few consensual documents tackling the psychological approach which should be used to talk about the overweight issue to these adolescents. Our goal here is to analyse how overweight and obese adolescents feel and cope with it when the topic is addressed by the general practitioner, in order to better identify the psychological approach that the latter should follow when talking to teenagers. Materials and methods: This is a qualitative study carried out through semi-structured, one-on-one interviews. Results: The teenagers' reactions when the practitioner would address the issue of their “new body” during consultations would differ. One persistent element in those interviews was the distress resulting from the overweight. The way this distress was expressed would differ from one individual to the other. Exchanging with the general practitioner about this suffering, more specifically the distress resulting from being judged in society, was tremendously important for them, provided that the practitioner would respect a number of requirements. Teenagers would sometimes break away from medical care of overweight because of the demeanor they would perceive from the practitioner, which, according to them, was not suited to their issue. Discussions: Discussions between practitioner and teenager account for a major stake in the teenager's participation in medical care. The practitioner must keep in mind that the teenager's body is a new element in their life and that they must learn and gradually explore it. It is important that the practitioner asks the teenager about the mental and social suffering resulting from this body, more specifically from social judgements. Conclusion: In order to take care of those overweight and obese adolescents better, it is important to assess the teenager's degree of mental distress on the one hand, as well as to take the social and family environments into account, and finally to create another medical approach and involve the teenager in their medical care

Introduction: During the general practice internship, direct supervision (SD) is the phase where the general practitioner supervisor (GPS) observes the student performing the consultations. It is rich in information for the GPS and learning for the resident through feedback on what has been achieved. In France, few studies have focused on what the residents were experiencing during this phase. Objective: To explore how general practice residents experienced the direct supervision phase during their level 1 internship. Method: A qualitative survey by 13 semi-directed interviews conducted between April and October 2019 with general practice resident who completed a general practice level 1 internship during the 2018-2019 winter semester. Recording interviews, complete transcription with anonymization and then analysis in a grounded theory. Results: Some interns were apprehensive about the SD phase for fear of being judged by their GPS. While performing the consultation under the watchful eye of their GPS was frightening. Residents, discovering general medicine, had to familiarize themselves with the environment of their new work, learn how to work besides a GPS and adapt themselves with their patients. Thanks to the support and confidence of the GPS, the residents felt more comfortable and gained confidence. The analysis revealed several criteria that promoted a good experience, such as preparing the residents by explaining the progress of the phase, the withdrawal of the GPS during the consultation and the achievement of feedback. Conclusion: Despite the perceived difficulties, SD seemed to be an enriching and indispensable phase. Simple advice would improve students' experiences

Psychological health at work for health care providers is an important issue since they are directly involved in quality of patients care. Several indicators are used to evaluate psychological health at work. They are influenced by managerial and organizational factors. In adult onco-hematology, it has been shown that participative management improves quality of life of health care providers. On the other hand, in pediatric oncology, no study had been reported between managerial and organizational factors in psychological health at work of health care providers and quality of patients care. Our study points out that psychological health of health care providers in pediatric oncology is linked to organizational factors (organizational justice, Perceived organizational support) and managerial factors (perceived autonomy support, transformational leadership). Moreover, some of these factors has been associated with quality of care perceived by health care providers. No relation between dimensions of quality of care perceived by children and psychological health at work has been found. This study shows an association between participative management and psychological health at work. These results emphasize the important role of managers, of participative management and of organization in the psychological health at work of health care providers but, also, in the children's quality of care
Santé mentale et burnout des radiologues en région Centre-Val-de-Loire : état des lieux et facteurs de risques by Charlotte Philibert( )

Objective Carrying out a mental health and burnout inventory among radiologists in the Centre Val-de-Loire region, understanding the risk factors for burnout and poor mental health. Materials and methods An anonymous questionnaire sent by email to all radiologists in the region. In this study, burnout, anxiety and depression were studied using validated scales. Results Among the 109 radiologists from all sectors who responded (out of 247 asked), 67% of radiologists were considered to be in burnout, including 14% in severe burnout, 45% had a high anxiety rate and 23% were considered to be depressed. The main risk factors found were: overwork, lack of sufficient time to meet patients, pressure from imaging requests and lack of recognition. The scores were considerably worse in the hospital sector. Conclusion The mental health scores were very worrying and more negative than for most other medical specialties. It is important to know the risk factors for poor mental health and their link with working conditions to be able to propose solutions aimed at improving personal satisfaction and accomplishment at work
Etude fonctionnelle et phénotypique des effets de l'acide tout-trans rétinoïque sur les cellules CD34+ médullaires humaines by Olivier Hérault( Book )

Les rétinoïdes sont bien connus en hématologie comme agents différenciants agissant à des niveaux assez matures de la cascade hématopoïétique. Ils forcent la différenciation in vitro de bon nombre de lignées cellulaires matures, et constituent le traitement de référence des leucémies aiguës promyélocytaires. A l'inverse, il existe très peu d'informations concernant les effets de ces rétinoïdes à des stades plus immatures de l'hématopoïèse humaine adulte normale. L'objectif de cette thèse a donc été de caractériser les modifications phénotypiques et fonctionnelles des cellules CD34+ médullaires adultes, induites par des doses pharmacologiques d'acide tout-trans rétinoïque (ATRA), le plus actif des rétinoïdes. Nous avons caractérisé la morphologie cellulaire, le phénotype membranaire, la capacité clonogénique, le cycle cellulaire et le taux d'apoptose des cellules CD34+ avant et après exposition à l'ATRA. La qualité de l'ATRA utilisé a été vérifié en étudiant ses effets sur une cible bien caractérisée, la lignée HL-60 et nous avons développé une méthode cytofluorométrique originale de mise en évidence de l'apoptose dans une sous-population cellulaire utilisant un seul laser
