El objetivo del presente estudio fue valorar la utilidad del tilt test en pacientes con sincope de origen desconocido. Entre enero de 1994 y setiembre de 1995 se realizo tilt test a 93 pacientes, edad promedio 59 anos, 46 por ciento de sexo masculino, con diagnostico de sincope de origen desconocido y a 30 sujeitos control, edad promedio 53 anos, 50 por ciento de sexo masculino. El estudio consistio en una primera etapa de reposo en decubito dorsal de 15 minutos para obtener parametros basalaes y una segunda etapa de inclinacion a 80 grados durante 30 minutos. La tension arterial (TA) y la frecuencia cardiaca fueron monitoreadas cada minuto durante el procedimento. El test se considero positivo cuando el paciente presentaba caida de la TA sistolica> 30 mmHg. (vasodepresor), bradiasistolia (cardioinhibidor), o respuesta mixta, acompanada de sintomas. El tilt test fue positivo en 31 de 93 pacientes. (33 por ciento). Diecisiete pacientes (55 por ciento) presentaron respuesta vasodepresora, 3 pacientes (9 por ciento) cardioinhibitoria y 11 pacientes (36 por ciento) respuesta mixta. Los sintomas fueron presincope en el 62 por ciento, sincope en el 19 por ciento y en los restantes pacientes. (19 por ciento), mareos, disnea o perdida del equilibrio. Todos los pacientes se recuperaron en forma espontanea al retornar a la posicion supina. Ningun sujeto del grupo control desarrollo respuesta anormal o sintomas durante el estudio (sensibilidad: 33 por ciento y Especificidad: 100 por ciento). Se concluye due el tilt test es un metodo diagnostico seguro y efectivo para identificar el sustrato neurocardiogenico en pacientes con diagnostico de sincope de origen desconocido. (AU)

Los cardiodesfibriladores son una terapeutica eficaz en el tratamiento de las arritmias ventriculares malignas. Nuestro trabajo tiene por objetivo valorar si los telefonos celulares de tipo analogico (disponibles en nuestro pais) producen interferencia electromagnetica clinicamente significativa en pacientes con dispositivos implantables. Se evaluaron 25 pacientes portadores de cardiodesfibriladores y solo un paciente presento interferencia electromagnetica sin repercusion clinica manifiesta por ausencia de registro en el marcador de canales y en el electrograma endocavitario. Se concluye que los telefonos celulares producen interferencia electromagnetica en los cardiodesfibriladores, pero solo en ocasiones particulares podrian tener relevancia clinica. Asimismo se recomienda llevar y utilizar los telefonos del lado contralateral al del implante (AU)

Abstract : Brugada syndrome is a form of inherited arrhythmia syndrome characterized by a distinct ST-segment elevation in the right precordial leads. Brugada phenocopies are clinical entities that present with an electrocardiographic pattern identical to Brugada syndrome and may obey to various clinical conditions. We present a case of a suicidal attempt using a high dose of propafenone causing a Brugada-type electrocardiographic pattern. Is this a Brugada syndrome case, a Brugada phenocopy or something else?

Un joven de 25 anos sin enfermedades previas conocidas, se colapsa durante un medio maraton en el mes de diciembre (temperatura ambiente 36[vbar]C). Luego de su admision presento hipertermia central, arritmia ventricular compleja, deterioro neurologico, rabdomiolisis, anuria y coagulacion intravascular diseminada, falleciendo en el termino de pocos minutos. La muerte subita en el atleta plantea un desafio desde el punto de vista medico como social. Las enfermedades que la causan se suelen agrupar de acuerdo con su prevalencia -dada por la edad del sujeto-, siendo notoria en muchas casuisticas la ausencia de una entidad potencialmente letal (pero rapidamente reversible) como es el golpe de calor. Este es un cuadro de instalacion subita, debido a una gran produccion de calor y a la dificultad para su disipacion. Si ambas condiciones persisten, se genera fallo multiorganico y finalmente colapso cardiovascular. La importancia del diagnostico precoz se evidencia en que la normalizacion rapida de la temperatura central elimina dichas complicaciones, mientras que la demora en reconocer el cuadro puede producir secuelas graves o incluso la muerte del sujeto. En la discusion se exponen las diferencias con el golpe de calor clasico y las medidas que deben adoptarse (tanto medicas como generales) para modificar la evolucion natural de esta entidad.(AU)

Abstract : Alcoholic beverages have been consumed for thousands of years, attracting great human interest for social, personal, and religious occasions. In addition, they have long been debated to confer cardioprotective benefits. The French Paradox is an observation of a low prevalence of ischemic heart disease, with high intakes of saturated fat, a phenomenon accredited to the consumption of red wine. Although many epidemiological investigations have supported this view, others have attributed it to beer or spirits, with many suggesting that the drink type is not important. Although excessive consumption of alcoholic beverages is commonly regarded to be detrimental to cardiovascular health, there is a debate as to whether light-to-moderate intake is cardioprotective. Although there is extensive epidemiological support for this drinking pattern, a consensus has not been reached. On the basis of published work, we describe the composition of wine and the effects of constituent polyphenols on chronic cardiovascular diseases
Intermittent preexcitation indicates "a low‐risk" accessory pathway: Time for a paradigm shift?( )

Abstract : We report three patients with intermittent loss of the preexcitation pattern in the ECG that had undergone an electrophysiological study. Despite apparently poorly conducting accessory pathway (AP), in each case a fast anterograde conduction, either during spontaneous atrial fibrillation or during incremental atrial pacing (on isoproterenol) was documented; shortest preexcited RR intervals of 200–240 ms were observed. We review the literature and conclude that intermittent preexcitation observed on resting 12‐lead ECG lacks sufficient specificity for the diagnosis of an AP with long refractory period and cannot be considered a substitute for electrophysiological study in patients with this electrocardiographical phenomenon
Erythromycin, QTc interval prolongation, and torsade de pointes: Case reports, major risk factors and illness severity( )

Objectives: Erythromycin is a macrolide antibiotic that is widely used for various infections of the upper respiratory tract, skin, and soft tissue. Similar to other macrolides (clarithromycin, azithromycin), erythromycin has been linked to QTc interval prolongation and torsade de pointes ( TdP ) arrhythmia. We sought to identify factors that link to erythromycin-induced/associated QTc interval prolongation and TdP . Methods and Results: In a critical evaluation of case reports, we found 29 cases: 22 women and 7 men (age range 18–95 years). With both oral and intravenous erythromycin administration, there was no significant relationship between dose and QTc interval duration in these cases. Notably, all patients had severe illness. Other risk factors included female sex, older age, presence of heart disease, concomitant administration of either other QTc prolonging drugs or agents that were substrates for or inhibitors of CYP3A4. Most patients had at least two risk factors. Conclusions: On the basis of case report evaluation, we believe that major risk factors for erythromycin-associated TdP are female sex, heart disease and old age, particularly against a background of severe illness. Coadministration of erythromycin with other drugs that inhibit or are metabolized by CYP3A4 or with QTc prolonging drugs should be avoided in this setting
