Les contributions reunies ici s'interessent aux modalites de prise en compte et aux conditions de manifestation des changements climatiques a l'echelle locale a partir de reflexions complementaires sur les processus d'institutionnalisation et l'evolution des referentiels d'action, ainsi que sur des etudes de terrain. Des analyses centrees sur les politiques publiques specifiquement dediees a l'enjeu des changements climatiques (etudiees de facon generale en France et plus particulierement aux niveaux des regions et de deux metropoles - Paris et Lyon) sont croisees avec l'analyse de situations ou l'enjeu du climat arrive par la marge, via la gestion des risques dans les milieux littoraux ou par le biais de mobilisations sociales (mouvement des Transition Towns). Cette diversite de points de vue permet de nourrir une discussion sur l'evolution de concepts cles associes a l'action territoriale, tels ceux de vulnerabilite, d'adaptation et de durabilite, potentiellement remis en cause par l'imperatif d'agir localement sur les changements climatiques. Ce qui conduit a interroger en quoi la question du climat vient changer - ou au contraire, faire perdurer - les assises de l'action locale et environnementale et les preceptes du developpement durable

This thesis deals with conflicts and territorial issues. It analyses institutional informative procedures specific to waste public policy, considered as public policy instruments, in order to estimate their ability to facilitate local waste management. Using a public policy analysis approach, we aim to understand the expectations put on informative procedures as well as their implementation. Three points are developped : an analysis of the French domestic waste policy from 1975 to 2005, a local case study about the planning process and a conflict in Indre-et-Loire, and an empirical study in Centre region about local information and control committees around waste treatment plants

Dans le domaine de la gestion des déchets, les oppositions à l'implantation d'équipements (décharges, incinérateurs ou plateformes de compostage) se multiplient malgré le renforcement des normes environnementales et la mise en place de procédures d'information, de concertation et de participation. Cet ouvrage se propose d'étudier cet apparent paradoxe à partir de l'analyse de situations de conflit / participation dans trois contextes nationaux (en France, en Italie et au Mexique) caractérisés par le blocage des décisions publiques malgré une grande diversité de modalités d'intervention. Quels sont les impacts des procédures et des documents de planification? Quels sont les référents spatiaux et territoriaux mobilisés par les opposants? Quels sont les usages du droit? Sur la base d'une analyse des dimensions temporelles, territoriales et juridiques des situations étudiées, les travaux réunis ici interrogent les relations entre conflit et concertation. Contemporary societies are affected by increasing conflicts over urban development projects. In the waste management sector, opposition to the construction of waste treatment plants (landfill sites, incinerators or composting facilities) has grown in spite of more stringent environmental standards and the provision of information, collaboration and participation procedures. This book examines this apparent paradox by analysing conflict/participation situations in three countries (France, Italy and Mexico) characterised by blocking public decisions despite a wide variety of intervention methods. What is the impact of planning procedures and documents? What spatial and territorial references do the opponents use? How is the law used? These studies analyse the temporal, spatial and legal aspects of the situations to examine the relationships between conflict and communication

Dans les villes contemporaines, les situations dans lesquelles des résidents s'opposent à l'implantation d'un équipement, protestent contre des nuisances, s'organisent pour obtenir la préservation d'espaces chargés de valeurs environnementales ou patrimoniales sont souvent considérées comme des manifestations d'une tendance généralisée au repli sur l'espace de résidence qui contribuerait à la fragmentation urbaine. Ce livre propose une perspective différente permettant de construire une réflexion sur la productivité sociale et territoriale de ces situations d'action. Quatre hypothèses d'interprétation des effets des conflits de proximité sont ici développées, ceux-ci peuvent être analysés comme des processus de territorialisation, comme des modalités d'actualisation locale du droit ou de production d'un espace public (politique) intermédiaire et comme des transactions sociales. Il s'agit donc de considérer les conflits de proximité dans leur dimension d'action collective et d'interroger l'apparent paradoxe entre renforcement du cadre juridique urbain et environnemental, approfondissement des procédures de participation et multiplication des conflits. Testicular ultrasensitive Doppler preliminary experience: a feasibility study( )

Background Ultrasensitive Doppler is a novel non-invasive ultrasound (US) Doppler technique that improves sensitivity and resolution for the detection of slow flow. Purpose To investigate the feasibility of ultrasensitive Doppler (USD) for testicular disease diagnosis, using both qualitative and quantitative results. Material and Methods This prospective study was conducted in 160 successive men referred for scrotal US including B-mode and conventional Color-Doppler. A new USD sequence and algorithm dedicated to academic research were implemented into the US system. The quality criterion for a successful examination was the detection of well delineated intratesticular vessels. Qualitative USD results were described in terms of tumor vascular architecture and flow intensity for different pathologies for 41 patients. The testicular vascularization (TV), defined as a vessel’s surface ratio, was quantified using customized MATLAB® software and compared in azoospermic and normal patients. Results USD was acquired successfully in 153/160 patients (95.6%). The tumor vascular architecture differed depending on the nature of the tumors. Leydig cell tumors exhibited mostly circumferential vascularization, while germ cell tumors exhibited straight vessels through the tumors, or anarchic vascular maps. USD improved the diagnostic performance of testicular Doppler US in a case of incomplete spermatic cord torsion and acute epididymitis. The reproducibility of TV measurements established an interclass correlation of 0.801. Non-Klinefelter syndrome non-obstructive azoospermia patients exhibited a lower TV compared to normal patients, to Klinefelter syndrome, and to obstructive azoospermia patients (P < 0.002, P < 0.005, and P < 0.05, respectively). Conclusion Testicular USD can become a promising technique for improving US diagnosis of tumors, acute scrotum, and for determining infertility status

Non-palpable incidentally found testicular tumors: Differentiation between benign, malignant, and burned-out tumors using dynamic contrast-enhanced MRI( )

Abstract: Purpose: To evaluate qualitative, semi-quantitative, and quantitative parameters obtained by dynamic contrast-enhanced (DCE)-MRI for the characterization of histologically proven, non-palpable, incidentally found intratesticular tumors. Materials and methods: From 2006 to 2014, we included men with non-palpable, incidentally found testicular tumors on ultrasound, normal tumoral marker levels, referred for surgery. DCE-MRI data were analyzed retrospectively and independently by two radiologists blinded to the histological diagnosis. The visual enhancement patterns, time-signal intensity curves, shape of the curves (type 0–3), maximal relative enhancement (Peak), initial enhancement slope (IS), time to peak (TTP), as well as transfer constants Ktrans and Kep were compared between the tumors. The interobserver correlation was evaluated. Receiver Operating Characteristic (ROC) curves and areas under the curve (AUC) were extracted. Results: Thirty-one patients (mean age of 37.3 years) were included. Tumor mean size was 1.2 ± 0.77 cm (min = 0.3 cm, max = 2.8 cm). Regarding the histology results, three groups were defined: Twelve stromal "benign tumors" (BT) exhibited more type 2 and type 3 curves than 12 "malignant tumors" (MT) and 7 "burned-out tumors" (BOT) (p < 0.0001). BT had a higher peak (96 vs. 54 and 17%), shorter TTP (215 vs. 412 and 692 sec), higher IS (73 vs. 12 and 2 arbitrary units), higher Ktrans (255 vs. 88 and 14 min −1 *1000) and higher Kep (554 vs. 159 and 48 min −1 *1000) than MT and BOT, respectively (p < 0.0001, p = 0.0003, p < 0.0001, p < 0.0001 and p < 0.0001, respectively). The agreement coefficient values and the AUC extracted after gathering MT with BOT varied from 0.83 to 0.96 and from 0.868 to 0.978, respectively. Conclusion: DCE-MRI may assist in differentiating between benign intratesticular stromal tumors, malignant and burned-out tumors
