The book provides an overview of the entire field of sleep medicine, from the basics of sleep physiology to the diagnosis and treatment of specific sleep disorders. Abstract: Background<br>The standard therapy for obstructive sleep apnea (OSA) is nighttime positive airway pressure (PAP). Generally, it has few side effects and represents a long-term treatment. However, not every patient is able to tolerate or accept the therapy or is willing to comply long-term. Other patients even have to terminate treatment for objective reasons.<br><br>Objectives<br>There are no clear terms defined yet in clinical guidelines for the noncompliance of PAP therapy. The aim of our article is to introduce standardized definitions that may be used as a reference for clinical practice and scientific approaches.<br><br>Materials and Methods<br>A board of experts has extensively studied the literature, definitions and background information regarding the topic. They considered different types of noncompliance or termination of PAP treatment and thus formed categories and definitions.<br><br>Results<br>These are the categories of PAP noncompliance as proposed by the board of experts: PAP non-acceptance, PAP incompatibility, PAP intolerance, PAP failure, and PAP discontinuance.<br><br>Conclusion<br>These categories, including definitions, may serve as a foundation for standardized management of OSA patients that need to be prepared for PAP therapy

With 2 - 4%, OSAS has a high prevalence in the population. It leads to a reduction in the quality of life (e.g. daytime sleepiness) and secondary diseases (e.g. arterial hypertension). OSAS is a population-relevant disease. Various therapeutic options are available. Continuous positive airway pressure ventilation during nighttime is the gold standard for severe forms of the disease. The early diagnosis and initiation of therapy could prevent the occurrence of secondary diseases associated with OSAS and improve the quality of life. The reliable diagnosis is made with the AHI, which can be determined as part of a polysomnography in the sleep laboratory or by ambulatory diagnostic using polygraphy. Some authors assume, that there is a high number of unreported OSAS cases. It therefore makes sense to optimize the screening of OSAS, in order to identify those affected at an early stage and to initiate further diagnostics / therapies. In this effort, publications have been released since the early 1990s, that focus on possible predictors of OSAS and / or present prediction models. With regard to their predictive value, the results of a clinical ENT examination were also investigated. Initially the static parameters of a clinical ENT examination were considered to be of great importance in the development of OSAS. Abstract: There are several differential diagnoses of unilateral sinus disease. One of these is inverted papilloma (IP) of the maxillary sinus, which is a common benign tumor with a substantial rate of malignant transformation. In general, endoscopic endonasal techniques for addressing the tumor are favored nowadays instead of classical external approaches. The aim of this retrospective study was to investigate the long-term outcome of inverted papilloma treated endoscopically via the prelacrimal approach. We reviewed 17 patients with primary or recurrent IP of the maxillary sinus that were treated via the prelacrimal endoscopic endonasal technique. After a median follow-up period of 45.9 months (3.8 years), none of the 17 included patients showed signs of recurrent disease and no serious complications were reported. Hypoesthesia of the incisors was reported by four patients and was resolved with time in one. All of the maxillary sinuses could be fully visualized with the flexible endoscope. IP is an important differential diagnosis in the clinical finding of unilateral nasal polypoid lesions. The prelacrimal approach is an effective and safe method in the treatment of IP with limited patient morbidity

Corneal endothelium after infrared laser exposure - evaluation of techniques to study corneal lesions by United States( Book )

Specimens of corneal endothelium of adult rhesus monkeys were evaluated for alterations by one of three staining techniques. One cornea from each animal had been exposed to carbon dioxide laser radiation (at 10.6 micrometer, 100 msec, 65, 45, or 35 watts/sq. cm, spot diameter of 3.2 mm at the 1/e intensity points) and removed surgically two hours after exposure. The other eye of each of the 18 monkeys served as controls (they were not exposed to laser radiation). Six specimens, two at each exposure, were stained with trypan blue (TB); nine specimens, three at each exposure, were stained with tetrazolium salt medium (TNBT); and three specimens, one at each exposure, were stained with hematoxylin. Each control cornea was stained with the same as the matching specimen. All specimens were processed for histologic examination by embedding in Epon. The combined use of TB and TNBT staining procedures allows detection of subtle alterations of the corneal endothelium at irradiances as low as 35 watts/sq. cm of CO2 laser (10.6 micrometer) radiation for a 100 msec exposure. At irradiances from 35 to 65 watts/sq. cm, a progressive increase in the incidence of altered cellular viability and enzyme activity is induced. At these techniques, in combination, are applied, the response of the corneal endothelium to infrared laser radiation and its dose dependence can be described more accurately. (Author)
