More than 10 years have passed since the memorable symposium on "Myocardial Contractility," edited by R.D. Tanz, F. Kavaler and J. Roberts (New York and London, Academic Press, 1967). PathogeneSis of myocardial failure still involves many questions. The latest scien tific findings on fundamentals of myocardial contraction encouraged us to organize this international symposium held in Rottach-Egern at Tegernsee (Germany), June 17 to 19, 1976 sponsored by the European Society of Cardio~ogy. It seemed appropriate to assemble prominent workers in this field in an attempt to correlate their respective in formation on cardiac function. In this connection it must be remembered that our present understand ing of the cardiovascular system and today's therapeutic and preventive measures are the fruits of yesterday's research. Further progress in this field will be conditioned by various circumstances: to win highly motivated creative people for clinical research, to mediate time and contacts for their learning new methods, and to provide adequate faci lities for scientific work in our hospitals. On the occasion of the inauguration of the Department of Heart Surgery at the Rehabilitation Center in Bad Krozingen, we held under the aus pices of the European Society of Cardiology an International Symposium on March 17 to 18, 1978 with the topic: . "Coronary Heart Surgery - a Rehabilitation Measure" This book contains the papers given at that time. The editors of this book, together with the other members of the sci entific committee - H. Denolin from Brussels, Ch. Hahn from Geneva, and F. Loogen from DUsseldorf - aimed at selecting controversial top ics as well as speakers representing different viewpoints. Thus avoid ing portrayal of an image of a world of coronary bypass surgery in which all problems have found a solution. In keeping with the topic of the meeting, special emphasis was drawn to the long-term functional results of aortocoronary bypass surgery; symptomatic imprQvement be ing, in most patients, significant enough and lasting for at least some years, thus allowing the patients to return to work. However, a combination of social, economic, and psychological factors may hamper optimal rehabilitation, as can be seen from some of the following papers. The incorporation of heart surgery into a rehabilitation center - being the motif of this symposium - may hopefully stimulate a global approach to cardiac rehabilitation which includes surgical, medical, psychological, vocational, and social rehabilitation mea sures. In recent years there have been major advances in the fields of cardiovascular nuclear medicine and cardiac magnetic resonance imaging. In nuclear cardiology more adequate tomographic systems have been designed for routine cardiac use, as well as new or improved quantitative analytic software packages both for planar and tomographic studies implemented on modern state-of-the-art workstations. In addition, artificial intelligence techniques are being applied to these images in attempts to interpret the nuclear studies in a more objective and reproducible manner. Various new radiotracers have been developed, such as antimyosin, labeled isonitriles, metabolic compounds, etc. Furthermore, alternative stress testing with dipyridamole and dobutamine has received much attention in clinical cardiac practice. Magnetic resonance imaging is a relative newcomer in cardiology and has already shown its merits, not only for anatomical information but increasingly for the functional aspects of cardiac performance. This book covers almost every aspect of quantitative cardiovascular nuclear medicine and magnetic resonance imaging. It will assist the nuclear medicine physician, the radiologist, the physicist/image processing specialist and the clinical cardiologist in understanding the nuclear medicine techniques used in cardiovascular medicine, and in increasing our knowledge of cardiac magnetic resonance imaging

It was a great pleasure for us to welcome so many experts from all over the world to our symposium in Aachen. We are also pleased - and you can attribute this to my own vanity - with the success and acceptance of the concept of myocardial reperfusion and revascularization - specifically selective intracoronary lysis followed either by PTCA or bypass surgery - which we have been pursuing since 1979. But after the dramatic immediate effects of the first attempts, which you have to experience yourself, we did not expect it to be any different. We decided against performing a randomized study in which every patient is catheterized, and thrombolytic therapy only given to some of the patients, for which we have been criticized at times. At the previous symposium in Aachen in 1983 on the topic of thrombolysis for acute myocardial infarction, in the final session several speakers were asked directly about this topic, and replied that if they were patients they would not want to be randomized into the placebo group. In the meantime positive results from large, randomized studies have been recorded, which are presented in this volume. I mention in particular the Western Washington Study from Dr. Kennedy and the Interuniversity Study in Holland. Aachen, a relatively small city with 250,000 inhabitants, provided the opportunity to treat a relatively large patient population with acute myocardial infarction
