Demographic and utilization date are used to project the need for physicians to provide care for the elderly over the next 50 years. The quantitative estimates are coupled with qualitative analyses leading to recommendations for means to deliver the needed services. Estimates are that the United States will require about 8,000 geriatricians by 1990, based on a proposed redistribution of physician training effort. The shortfall of physician manpower in geriatrics will demand that physicians share the care of elderly patients with nurse practitioners, physicians' assistants, and social workers. The solution to the problem of medical care for the elderly lies in developing physician leaders who can work with other professionals to develop an integrated approach. A substantial influx of resources will be needed to establish the academic milieu necessary to recruit and train the number of geriatricians required. A series of recommendations are offered for upgrading the professional standing of geriatrics, strengthening geriatric training, and enhancing the efficiency and quality of medical care for the elderly.
