Omosessualità, maschile e femminile, sadismo, masochismo, sadomasochismo, esibizionismo, feticismo sono stati considerati, fino a pochi decenni or sono, come fatti degenerativi, come mostruosità, per cui l'istinto sessuale , invece di manifestarsi nelle forme comuni all'uomo normale, assumerebbe aspetti strani, del tutto estranei alle finalità biologiche dell'istinto stesso. In questa tesi si inseriva, contro ogni coerenza, anche l'idea che le perversioni fossero invece il prodotto di una ricerca volontaria della novità nell'esercizio dell'attività sessuale. I nuovi concetti introdotti dalla psicoanalisi nell'interpretazione della sessualità hanno fatto apparire il problema in una luce del tutto nuova, mostrando come il comportamento sessuale "normale" dell'uomo sia il risultato di uno sviluppo armonico e selettivo di tutta una molteplicità e varietà di sensazioni e di impulsi che nel bambino coesistono in uno stato caotico e embrionale. Lo sviluppo dell'istinto sessuale, l'organizzazione dei suoi elementi parziali, sono in larga misura preordinati: su un tale sviluppo influiscono numerosi fattori di natura diversa, che possono affrettarlo, ritardarlo, o dargli elementi particolari. Si ha una "perversione" quando, per motivi di vario genere, si attua un distacco dalla finalità fondamentale dell'istinto sessuale, ossia quando si dà un rilievo anormale a funzioni o elementi di tutto secondari escludendo il congiungimento sessuale normale, o quando all'oggetto normale dell'atto sessuale si sostituisce un oggetto diverso.
