The purpose of this presentation is to describe and illustrate the mechanisms which regulate the urinary concentration and dilution and the physiological response to an excess or lack of water. This objective is achieved with the aid of diagrams, drawings and structural formulas. A pre- and posttest are provided and the viewer is asked questions throughout the program. Correct answers are provided. In this presentation the transport of solute from the lumenal fluid in the loop of Henle is described. The effects of the presence of excess water and the lack of water on this solute transport is then discussed. The roles of the antidiuretic hormone and the countercurrent multiplier are described in detail. Factors responsible for the formation of an appropriately diluted or concentrated urine are identified and described. These factors include the delivery of adequate volumes of salt and water to the ascending loop of Henle, active reabsorption of salt at the ascending loop of Henle, appropriate control of the release of the antidiuretic hormone and the transport of sodium salts out of the fluid in the collecting duct when excess water is excreted. Formulas are then presented which can be used to measure osmolar clearance and free water clearance

The purpose of this teaching program in renal physiology is to describe and illustrate the fluid compartments of the body, osmolarity regulation within the nephron and countercurrent multiplication and its physiological application. This objective is achieved with the aid of charts, diagrams, drawings, and graphs. The viewer is asked to stop the program at intervals and answer questions. Correct answers are provided. An entry test and posttest are also provided. According tothis presentation the total body water is distributed into three functionally distinct compartments: the intracellular fluid, the extracellular fluid and the plasma. The distribution of sodium and potassium in these compartments is discussedand the water permeability of the cell membrane is described and illustrated. The program then discusses and illustrates the physiological problem of concentration regulation and the structure of the nephron and its adaptation to this physiological problem. The anatomy and physiology of the juxtamedullary nephron is then reviewed. The transport and absorption of the glomerular filtrate is then described in detail. The establishment and multiplication of the gradient formationare described and illustrated. The important features of the countercurrent multiplier are identified and described including the counterflow of fluid resulting from the anatomic arrangement and the exchange of solute resulting in multiplication of the small gradient along the axis of the loop of Henle. Factors which influence the concentration gradient are then identified and discussed

Pathogenesis, evolution, anatomic and clinical manifestations of glomerulonephritis are discussed. The current classification into immune complex, antibasement membrane, membraneous and membrane proliferative glomerulonephritis and lipid nephrosis is covered. Chronic glomerulonephritis is mentioned. The alteration of physiology that may accompany glomerular disease is explained somewhat simplistically. This program is highly recommended
