The purpose of this presentation is to demonstrate step-by-step a surgical procedure for the treatment of a bladder diverticulum which does not necessitate the removal of the entire sac of the diverticulum. This objective is achieved with the aid of x-rays. In this presentation after a midline incision is made the bladder is opened and the contents evacuated. The incision in the bladder is enlarged, the ureteral and diverticular orifices are identified, and the contents of the diverticulum are evacuated. A finger is placed through the diverticular orifice. The peritoneum is reflected from the bladder wall down to the diverticular orifice. The diverticular sac is identified and the lateral bladder wall is exciseddown to and through the diverticular orifice and laterally through the anterior wall of the diverticulum. An incision is made through the bladder mucosa as it reflects from the bladder into the diverticulum. The mucosa is then stripped from the interior of the diverticulum beginning at the mucosal incision at the diverticular orifice and should be removed for a distance of at least two centimeters from around the diverticular orifice. The diverticular orifice and incision in thelateral bladder wall are closed in two layers. Drains are inserted in the diverticular cavity and the prevesical space. A No. 24 Foley catheter is left in the urethra. The procedure is demonstrated on a second male patient to emphasize the steps in the procedure
