OBJECTIVE: To define epidemiologic relationships to determine the prevalence and potential risk factors for nasopharyngeal colonization by antibiotic-resistant pneumococci, their serotypes and their antibiotic susceptibility patterns in children attending a daycare center (DCC). MATERIAL AND METHODS: A prospective cohort study was conducted among children (n = 53) attending the DCC at Hospital Infantil de MUxico Federico Gomez, which is staffed by 20 employees. Patients were enrolled in the study during a two-year period from September 1997 to September 1999. All the participants were followed prospectively, swabbing them every four months. The strains recovered were typed and screened for susceptibility to several antibiotics. The daycare records were reviewed also. Odds ratios and fisher's exact test: or chi square test of significance were computed from contingency tables as appropriate. Exact 95 confidence intervals were computed for odds ratios. Data analysis was performed using Epi statistics program version 6.04 a. RESULTS: Pneumococci were recovered from 45/53 of the infants at one or more visits. A total of 178 isolates were carried. The carriage rate was 47. Only 7 adults acquired pneumococci during the study. Types 6, 14, 19 and 23 were prevalent and represented 77 of the total. Antibiotic-resistant strains were higher to penicillin and erythromycin. CONCLUSIONS: Children were frequent carriers of pneumococci, the rate of carriage was high in infancy and tended to decrease with age. The types commonly carried by children were the same as those causing invasive disease. There is a high proportion of carriers with antibiotic-resistant S. pneumoniae strains. Children who have had frequent antimicrobial courses are at particular risk.(AU)

El cloramfenicol fue introducido en la pratica clinica en 1949. Desde entonces ha probado su eficacia, por lo que continua siendo el farmaco de eleccion para infecciones comunes. El conocimiento de su farmacocinetica, ajuste de las dosis, via de administracion y duracion del tratamiento han reducido al minimo los efectos indeseables. Es de primera eleccion en infecciones sistemicas por Salmonella sp y S. typhi en caso de cepas resistentes de H.influenzae, N.meningitis y tal vez por S.pneumoniae. Ademas es una opcion para infecciones comunes como otitis media y conjuntivitis bacteriana en cepas aisladas de procesos infecciosos frecuentes y graves como septicemia, meningitis, neumonia, otitis, epiglotitis y conjuntivitis, brindan apoyo en la seleccion especifica de este medicamento (AU)

Los objetivos de este trabajo fueron la comparacion de un medio de cultivo selectivo y otro enriquecido para el aislamiento de N. gonorrhoeae en 128 pacientes con blenorragia y el reconocimiento de cepas inhibidas por la vancomicina en esa poblacion. El grupo estuvo comprendido por 35 mujeres y 93 hombres a quienes se les practico cultivo de secrecion endocervical y uretral, respectivamente. Las muestras fueron inoculadas por duplicado simultaneamente en el medio de Thayer-Martin (MTM) y en el medio de gelosa chocolate (GCH), para depues procesarse de acuerdo con los patrones establecidos para la identificacion de N. gonorrhoeae. A quellas cepas que solo crecieron en GCH al primoiaislamiento, e igualmente en una resiembra posterior en donde se incluyeron ambos medios, se consideraron sensibles a la vancomicina. Las cepas que crecieron en el MTM tuvieron una CMI a la vancomicina igual o menor al 1 mcg/ml. De la comparacion total de los medios de cultivo empleados, se obtuvo una diferencia significativa (p<0.02) que favorecio al medio selectivo; sin embargo, al evaluar tal ventaja de acuerdo al sexo de los pacientes, ya no se denoto valor estadistico. En cuanto a la frecuencia de cepas de gonococo sensibles a la vancomicina se hallaron 128 inhibidas por el MTM (11,7%), parametro que se juzgo lo suficientemente importante como para recomendar el uso de ambos medios de cultivo, sobre todo cuando se trata de documentar la presencia de N. gonorrhoeae en poblaciones de talo riesgo o en aquellas que asisten a las clinicas de enfermedades venereas (AU)

Epidemiology of drug resistance: The case of Staphylococcus aureus and coagulase-negative staphylococci infections by Ernesto Calderón Jaimes( )

OBJECTIVE: To study the activity of several antibiotics against Staphylococcus spp. MATERIAL AND METHODS: The study included 1209 strains of Staphylococcus spp. from two institutions; Instituto Nacional de PediatrYa (National Institute of Pediatrics) and Hospital Infantil de MUxico Federico Gomez (Mexico City Children's Hospital). Minimum Inhibitory Concentrations of all antibiotics were determined by the agar macrodilution technique and standard methods from the National Committee for Clinical Laboratory Standards. RESULTS: Resistance of S. aureus was 14.2 and that of coagulase-negative staphylococci was 53.4. The activity of different antibiotics is presented in detail. El estreptococo beta hemolitico del grupo B requiere de una identificacion simple, oportuna y confiable que coadyuve a la atencion medica de los pacientes. El objetivo de este estudio fue practicar la prueba rapida de CAMP en gota para la identificacion presuntiva del microorganismo y compararla con el CAMP tradicional, asi como con las pruebas especificas de coaglutinacion y aglutinacion en latex. De las cepas bacterianas beta hemoliticas, obtenidas de diversas fuentes: 26 fueron de estreptococco B, ocho de estreptococco A, cuatro de estreptococo D, dos de estreptococo C y nueve de L. monocytogenes. La concordancia entre las pruebas de CAMP fue absoluta para las cepas que dieron un resultado positivo (26 estreptococos B y nueve listerias). Las 14 cepas restantes con CAMP negativo pertenecieron a los otros estreptococcicos senalados. Al valorar esta informacicon exclusivamente para el estreptococo B, en relacion a las pruebas de identificacion serologica (definitivas), la especificidad y la sensibilidad del CAMP en gota fueron del 100% con valor predictivo positivo de 1. En conclusion, esta prueba rapida constituye una alternativa eficiente y economica, al alcance de cualquier laboratorio bacteriologico, para lograr la identificacion de Streptococcus agalatie (AU)

Los antigenos de la cepa RH de Toxoplasma gondii han sido analizados empleando una variedad de tecnicas de cromatografia y electroforesis. La solubilizacion de los antigenos de membrana usando la combinacion de los detergentes saponina y octilglucosido ha mostrado ser eficaz; de esta forma han sido detectados hasta 12 antigenos. Por medio de estudios de interacciones hidrofobicas y de inmunoelectroforesis con I**125 se ha demostrado que los antigenos 4, 5 y 6 son de origen intracelular. Los antigenos 4 y 5 son implicados en la produccion de antigenos circulantes, en analisis in vitro se ha demostrado que el antigeno 5 es secretado por los parasitos. Tambien se ha observado que el antigeno 6 es el de mayor importancia en la prueba de hemoaglutinacion indirecta y que el antigeno 12 es el componente mayor de membrana, su presencia es de importancia en la prueba del colorante. Se han estudiado las proteinas que constituyen la membrana del trofozoito y se observo que la proteina de mayor membrana es la P30. El anticuerpo monoclonal que inmunoprecipita a esta es sintetizado por la clona B; estos anticuerpos tienen efecto parasitocida y pueden ser relevantes en la produccion de una respuesta inmunitaria protectora (AU)

OBJECTIVE: To describe the antimicrobial activity of several antimicrobial agents against 97 clinical significant isolates of Enterococcus spp. MATHERIAL AND METHODS: During a 2-year prospective study at Instituto Nacional de Pediatria (National Institute of Pediatrics) in Mexico City. Ninety seven strains of Enterococcus spp. (60 E. faecalis and 37 E. faecium) were tested against 11 antibiotics. Susceptibility tests were performed with agar, according to the standards of the sNational Committee for Clinical Laboratory Standards (NCCLS). Isolates were screened for high-level resistance (HLR) to beta-lactams, aminoglycosides, glycopeptides and other antibiotics, as well as for vancomycin-phenotypes. Differences between proportions were evaluated with chi2 of Fisher exact fest. RESULTS: Overall resistance rates to the antibiotics tested were: 17/97 (17.5 percent) to penicillin, ampicillin, amoxicillin-clavulanate and imipenem. There was neither HLR nor beta-lactamase production; 74/97 (48.4 percent) were resistant to erythromycin; 60 percent to ciprofloxacin; 31/97 (32 percent) to gentamicin, and 55/97 (56.7 percent) to streptomycin. Seven strains were vancomycin-resistant enterococci (VRE), all of them identified as E. faecium; 5/7 with Van A and 2/7 with Van B phenotypes. All the isolates were susceptible to linezolid. The difference in susceptibility among species was significant. CONCLUSIONS: Mutidrug-resistant enterococci is a real problem and continuous surveillance is necessary. The microbiology laboratory is the first line of defense against the spread of multiantibiotic-resistan enterococci in the hospital environment . All the strains recovered should be tested for susceptibility to ampicillin, streptomycin, gentamicin and glycopeptides (AU)

Se estudiaron 1235 mujeres de tres poblaciones diferentes bajo un criterio homogeneo que permitiera seleccionar aquellas con vaginosis bacteriana. Se demostro que una de cada cinco pacientes (235/1235) cumprio con los criterios establecidos. Asimismo se correlaciono la presencia de Gardnerella vaginalis con el proceso, encontrando que la asociacion entre aislamiento y vaginosis en esas poblaciones vario del 50 a 85%. Se analizan los diferentes criterios de diagnostico, correlacionandolos entre si para una mayor prediccion diagnostica. Se menciona la posible participacion de otros germenes anaerobicos residentes en el area, como patogenos potenciales del proceso. Finalmente se hacen consideraciones sobre la utilidad de los 5-nitroimidazoles, en la reolucion del problema clinico y microbiologico (AU)

En los ultimos anos ha aumentado el interes en las nuevas quinolonas como agentes antimicrobianos. Diversos compuestos se han utilizado y se ha desarrolado un intenso estudio microbiologico, farmacologico y clinico con estos nuevos farmacos. Un informe previo en esta revista, fue dedicado a comunicar la actividad in vitro de algunos de estos compuestos, comparados con aminoglucosidos. Desde la aparicion de ese articulo, se ha tenido un considerable avance en el conocimiento de la farmacologia de las preparaciones tanto bucales como parenterales. En este trabajo se presenta un panorama lo mas completo posible, de los productos que en un futuro proximo podran estar en la disponibilidad del mercado nacional, de tal manera, que pueda orientarse el conocimiento y posicion de cada uno de ellos como son: norfloxacina, pefloxacina, ciprofloxacina, enoxacina y ofloxacina (AU)
