Introduction: Healthcare is an essential component in maintaining the well-being of any community. The practice of healthcare is constantly evolving as a result of advancements in scientific knowledge and technology. In order to maintain and improve the quality and safety of healthcare services, healthcare research and regulation must be continuously updated. This constant evolution is dependent on data and outcomes derived from research data are directly related to the quality of the data. Poor data quality can lead to inaccurate and misleading conclusions. Assurance of data quality should be part of any study design and the various methods used should be studied for efficacy. Background: In September 2008, California senate bill 891 established the California Elective PCI Pilot Program, also known as the Percutaneous Coronary Intervention California Audit Monitored Pilot with Offsite Surgery (PCI-CAMPOS) pilot program. This is a three year pilot program which allows 6 California pilot hospitals with offsite surgery to perform elective PCI procedures and collect data on these procedures. The ultimate goal of the program is to assess the safety of elective procedures performed in hospitals with offsite surgery as compared to those performed at hospitals with onsite surgery. The final Advisory Oversight Committee report on the pilot program was submitted to the California Department of Public Health in November 2013. This report includes the outcomes of the pilot program along with recommendations on the future of elective PCI in California hospitals with offsite surgery. This study also focused on assessing the quality of data collected during the pilot program. Methods: Data on consecutive PCI procedures is submitted to a central database by all 6 pilot hospitals. This database is managed by the program coordinating center at the University of California, Davis. All submitted data undergoes the central site audit process on an ongoing basis; evaluating for completeness, valid values, and the presence of illogical entries. Another 10% random and all major complications are selected for pilot site audit evaluating for accuracy of the submitted data by comparison with source data at the pilot hospitals by trained auditors. Out of the 3,773 cases performed during the 3 year enrollment period (2010-2013), 861 cases were reviewed using the pilot site audit process. These 861 cases were selected as the sample for data quality assessment. The data quality assessment involved calculation of percent completeness, percent agreement and Cohen's kappa statistic for selected variables. Results: Data quality assessment resulted in 109 of 126 assessed variables showing 100% completeness, 13 variables with completeness from 99.92%-97.98%, 2 variables with 93.44% and data for 2 variables was not available. A total of 2,767 pilot site audit changes were made out of 81,626 opportunities. Overall percent agreement for the 3 year enrollment period was 96.61%, with individual years 1-3 showing 96.49%, 95.72% and 97.80% respectively. All selected variables assessed for estimated Kappa showed "substantial" to "almost perfect agreement" with kappa values ranging from 0.61 to 1.00. Conclusions: Data quality of the PCI-CAMPOS database shows good overall completeness, high accuracy and high reliability. The experience, results and conclusions drawn from the PCI-CAMPOS pilot program are supported by the high quality of data. Data quality assurance plans are essential to medical research and clinical registries, without such programs in place data quality can only be assumed
