This report examines the ability of the nonpsychiatrist physician to provide mental health services in an effort to guide policymakers dealing with the question of whether nonpsychiatrist physicians can, or should, be used to offset the projected shortfall of psychiatrists. The maldistribution of psychiatrists is discussed in chapter 1 of this report. Chapter 2 describes the National Ambulatory Medical Care surveys conducted in 1980 and 1981 as the data source for this analysis, and explains the sample and procedures used to define mental health visits. Chapter 3 compares mental health office visits to psychiatrists and to nonpsychiatrist physicians along a variety of dimensions: diagnosis, reason for visit, patient demographics, geographic location, length of visit, etc. In chapter 4, multivariate techniques are used first to estimate the probability that a patient will see a psychiatrist, and then to estimate the probability that a psychotropic drug will be prescribed. Chapter 5 presents the conclusions that nonpsychiatrist physicians: (1) appear to be part of a hidden mental health network, providing services to patients who cannot or need not obtain specialized care; (2) seem to substitute drugs for time in their treatment of mental disorders; (3) are less likely than psychiatrists to prescribe combination drugs; and (4) are less likely than psychiatrists to order major tranquilizers or the more serious and potentially dangerous antidepressants.
