Epidemiological data has linked maternal exposure to birth control formulations with an increased risk of infant acute leukemia (IAL) involving MLL gene translocations Subsequent in-vitro studies suggested a role of estradiol (E2) in the generation of such rearrangements. In order to probe the action of E2 in generating such lesions, the ability of E2 to increase transcription of the MLL gene was assayed using TK6 lymphoblastoid cells. Real-time PCR studies showed that in this cell line, E2 was able to increase the level of MLL transcription, as well as, the transcription of common translocation partner genes MLLT3 and AFF1. In addition, chromatin conformation capture data showed that E2 treatment increased the frequency of MLL and MLLT3 co-localization. Treatment of TK6 cells with E2 also increased the frequency of MLL and MLLT3 fusion transcripts as detected by primers located in intron 9 MLL and intron 4 of MLLT3, and intron 13 of MLL and intron 8 of MLLT3. RNAi knockdown of AICDA, an enzyme with the ability to break DNA that has often been involved in mediating translocations, was able to inhibit the increase in MLL-MLLT3 fusion transcript formation observed upon E2 treatment. Additionally, ChIP showed an E2-dependent localization of AICDA to the MLL Breakpoint Cluster Region (BCR). Combined these studies show that concentrations of E2 that occur during pregnancy, or during use of oral contraceptives, have the potential to initiate MLL fusions through an AICDA-mediated mechanism, only a fraction of which may then proceed to facilitate the development of leukemogenesis
