This investigation arose from the discovery that a program which had taken on responsibility for newborn screening in Ontario was moribund, and that children were dying or becoming unnecessarily disabled because newborns were not being tested. The investigation report reviews the history of newborn screening in Ontario from 1965, when the province was a world leader in introducing universal testing for an inherited metabolic disease, to a decline in commitment to preventive medicine for newborns and bureaucratic stagnation in the 1990s & later. It summarizes efforts by medical advisors, the public, & members of Parliament to increase the scope of newborn testing and recent government initiatives to introduce tandem mass spectrometry testing for over 20 disorders. Reasons for the failure to properly administer a newborn screening system are discussed, such as budgetary indecision, lack of policy direction, inadequate leadership, government abdication of responsibility for a screening program, and lack of an appropriate sense of urgency among politicians & bureaucrats. The final part reviews lessons learned that are important both to the future direction of newborn screening and for government in general
