The authors examine the history of Portugal's drug problem over the past 75 years in light of the existing literature on drugs and drug abuse

RAND Europe has conducted a cost-effectiveness analysis of the Mentor portal on drug misuse prevention. The main question of this research is: To what extent can the Mentor portal contribute to the benefits and the effectiveness of drug abuse prevention?" Although a pure cost-benefit analysis is not possible, taking the costs of the portal as the fixed expenditures for implementing it, this can be compared to various measures of non-monetisable benefit and some monetisable effectiveness estimations. The study starts with a review of the effectiveness of provision of information via the Internet in order to estimate the extent to which a portal can contribute to better prevention care

This study aims to identify which medications deserve priority for clinical research in the short term. The authors found that a lot of knowledge is already available and needs dissemination rather than further research. The medication for ADHD, Ritalin is the best example of this. However, many medications have not been studied yet in children and among those, two research questions are identified: 1 - clinical research on the safety, effectiveness and long term effects of (amongst others) psycho-active substances, new anti-epileptics and gastro-intestinal medications for use in children 2 - dissemination of knowledge on several medications to paediatricians, general practitioners and pharmacists

"RAND Europe was requested by the Committee on Policy Research concerning Drugs and Medical Aids (Commissie Beleidsonderzoek Genees- en Hulpmiddelen--BOGH) of the Dutch Health Care Insurance Board (College voor Zorgverzekeringen--CVZ) to develop a method to prioritize the need for research on pharmaceutical combinations. The pilot study for the method was to examine combination disease-modifying anti-rheumatic drug (DMARD) treatment for rheumatoid arthritis (RA). If successful, the method could be employed by the BOGH to advise the Ministry of Health, Welfare and Sport about priorities for investigating combination drug treatments of other diseases. In response, RAND Europe adapted a validated tool for assessing medical effectiveness, the RAND/UCLA Appropriateness Method to identify promising combination DMARD treatments for rheumatoid arthritis (RA) whose value is not believed to be well-established by scientific evidence. The Dutch Health Care Insurance Board commissioned RAND Europe to conduct a study to identify the barriers to effective prescribing for children, with an emphasis upon the absence of good clinical research within those barriers. The goals of this study are threefold. The first is to bring to awareness and to assess the importance of barriers to optimal pharmacotherapy for children. Here, the authors take the term "barriers" in its broadest sense, to include shortcomings in knowledge, communications, individual caregiver and patient behavior, and the organizational structure of the health care delivery system. The second goal is to identify possible solution directions, or recommendations for change, that can reduce these barriers. These solution directions are also assessed in terms of their likelihood of reducing barriers. The final goal of the study is to assess the extent to which clinical research is a useful solution direction, and which aspects of such clinical research (specific drugs, clinical condition addressed, dosage, side effects, etc.) are most important. Financing problems, lack of scientific knowledge and lack of dissemination of existing knowledge were found to be the main problems in pediatric care in the Netherlands. Preventing venous thromboembolism : prophylactic options for patients at different risk levels( )

Venous thromboembolism (VTE) represents a frequent and serious disorder influenced by numerous factors, but no current risk assessment model has achieved practicability for individual patient profiling of VTE risk. Following a literature review, two international panels, of medical and surgical experts, respectively, provided iterative rating of odds ratios for predisposing risk factors and Likert-type ratings for exposing risk factors. These were combined into a 3-dimensional matrix (inherent predisposing risk, general predisposing risk, exposing risk) to achieve a 4-level overall risk assessment (low, moderate, high, very high). The method was validated by holistic ratings of simulated patient cases. The appropriateness of 11 different possible prophylactic therapies for patients at all levels of risk was assessed by the panel, following the RAND/UCLA Appropriateness Method. The results of the investigation have been summarized in a DecisionMatrix that may be used by individual practitioners for guidance in deciding whether or not to use prophylaxis, and if so, what prophylaxis to employ
