"Research design. Following a literature review of models of psychiatric outreach, a mixed methods survey of psychiatric outreach consultants was conducted. A multi-mode survey was designed to examine existing models of service delivery used by psychiatric outreach consultants. A total of 27 questionnaires on outreach practices were completed by 25 consultants between October 2008 and February 2009; the majority were affiliated with the Ontario Psychiatric Program (OPOP). In addition, two focus group discussions were conducted in Fall 2008, one with OPOP-affiliated consultants, and one with a combination of northern-based psychiatric outreach consultants (with and without an OPOP outreach program affiliation), and some OPOP administrators. Topics of discussion centred on five themes: The service delivery setting, challenges in the outreach practice, collaboration and teamwork, linkages with tertiary care centres, and enhancing the OPOP model."--Executive summary

"The Ontario Psychiatric Outreach Program (OPOP) and the Centre for Rural and Northern Health Research (CRaNHR) at Laurentian University collaborated on a multi-method, multi-phase research project. This report addresses the findings of one piece, the Family Health Team Survey. The purpose of this study was to understand how mental health services were being integrated within Family Health Teams (FHTs). This study focused exclusively on FHTs in smaller communities located outside the northern urban referral centres (NURCs), in the North East and North West Local Health Integration Network (LHIN) regions. Fourteen of 19 eligible FHTs participated in telephone interviews in the Fall of 2008. FHTs were provided in advance with the question guide, and asked to select a representative to participate in the interview. The semi-structured interviews elicited information on FHT membership and practice characteristics, integration of mental health and psychiatric services, and education and training needs related to mental health services. Interview transcripts were analyzed using a descriptive content analysis approach. A small number of unprompted themes were identified."--Executive summary

"This study responded to Health Force Ontario's interest in conducting an evaluation of the Kenora Wound Care Pilot Project. Using available administrative data and interviews with health care provider and client-family caregiver dyads, the evaluation examined the implementation of the integrated wound care model and assessed evidence about program successes and challenges. Overall, the collaborative wound care model which was developed was viewed as being successful, because it created effective partnerships between diverse health care agencies, fostered interdisciplinary collaboration and improved competencies. There also was agreement that the model could be transferred to other interdisciplinary care settings and family health teams, if resources were provided.
