Les multithérapies antirétrovirales ont entraîné une amélioration spectaculaire de l'espérance de vie des patients infectés par le VIH, conduisant au vieillissement de cette population. La fréquence des comorbidités chez les patients âgés suggère un vieillissement prématuré lié au VIH. L'objectif de l'étude SIGMA est de décrire la population infectée par le VIH, âgée de 60 ans et plus, suivie à Bicêtre. Les patients présentant des troubles neuropsychiatriques graves ont été exclus. A l'aide d'auto-questionnaires et d'entretiens, des données médicales, psychologiques et sociales ont été recueillies. Les performances cognitives ont été évaluées. Parmi les 66 patients recensés, 41 ont accepté de participer à l'étude. L'âge médian est de 67 ans, 68% des patients sont de sexe masculin. Tous les patients traités ont une charge virale indétectable au dernier suivi, et 85% un taux de CD4 supérieur à 350/mm3. Les comorbidités métaboliques et cardiovasculaires sont fréquentes : 44% de patients sont suivis pour une hypertension artérielle, 42% pour une dyslipidémie, 36% pour une insuffisance rénale, 17% pour un diabète et 17% pour une cardiopathie ischémique. Une lipodystrophie est notée dans le dossier médical chez 44%. La moitié des patients a des performances cognitives anormales d'après le NPZ3 (NeuroPsychological Z-score, regroupant Trail Making Test et WAIS). Aucun facteur de risque de trouble cognitif n'a pu être identifié dans notre étude. Une perte d'autonomie est observée chez 15% d'entre eux, sans lien avec les fonctions cognitives. La qualité de vie est globalement bonne et n'est pas différente des résultats observés chez des patients plus jeunes. Le nombre d'effets indésirables rapportés influence négativement l'état de santé perçu. Malgré un contrôle immunovirologique très satisfaisant, la moitié des patients SIGMA a des fonctions cognitives altérées. Ces résultats contrastent avec une bonne qualité de vie ressentie. Des études longitudinales sont nécessaires pour confirmer ces données préliminaires

The availability of highly active antiretroviral combinations now extends the life expectancy of patients infected with HIV, but was associated with the emergence of new causes of death.We retrospectively analyzed the causes of deaths among HIV-infected patients followed at Bicêtre hospital between 2003 and 2005. Our data were collected using the standardized form of the multicenter French study ANRS "Mortavic 2005”. Our dead patients were first compared to deaths in France, second to patients staying alive in our center. Predictive factors of deaths from liver and HIV-related diseases have been assessed.We identified 51 deaths, and 45 were analyzed among 1224 patients followed in 2005. The median age at death was 42 years, with a median duration of HIV infection of 11.2 years. The majority of patients received antiretroviral therapy before death (89%), with a median CD4 lymphocyte count of 159/mm3 and a median viral load 4.36 log10 copies/mL. Sixty-five percent of patients were smokers, 60% were co-infected with hepatitis B and/or C. In our center, viral hepatitis was the first cause of death (36%), followed by AIDS diseases (29%) and cancers not linked to hepatitis nor HIV (16%).. In addition, patients of foreign origin were overrepresented in our center (20%) and were more at risk of AIDS-related deaths. Patients infected by intravenous drug use (56%) and coinfection with hepatitis C (58%) were overrepresented in Bicêtre compared to the Mortavic study.Part of the deaths having occurred in our center could have been prevented by earlier screening or care during infection, by addiction cessation, and even more by antiviral treatment of hepatitis. Recommendations made by French HIV experts took into account these findings at the national level and encouraged the Health Agency to strengthen the implications of general practitioners in HIV infection. Their role in early diagnosis of HIV infection, global patient monitoring, prevention of co-morbidities and coordination of care need to be reinforced
