Le façonnage chirurgical du corps répond à une nécessité psychologique encore peu définie. Depuis quelques années, la demande de chirurgie esthétique et sa satisfaction subséquente sont l'objet d'investigations multiples. Des facteurs sont identifiés comme intervenant dans le processus. Mais on déplore un manque de conceptualisation, limitant par ce fait, les capacités explicatives du phénomène. Ainsi, une analyse qualitative et quantitative est menée auprès de 50 personnes avant et 6 mois après l'intervention (n=22). Elle s'inscrit dans une perspective multidimensionnelle où le contexte social, psychologique et émotionnel est mis en lien avec le discours. Nos résultats supportent la pertinence d'un modèle multifactoriel en étayant l'existence d'un mode de fonctionnement particulier de ces personnes. Différentes formes s'allient avec le niveau de satisfaction après intervention : élevé (n=12), moyen (n=7) et bas (n=3). Les capacités prédictives du comportement sont limitées, mais encouragent la mise en place de thérapeutiques, interface nécessaire entre le soma et la psyché, dont l'alliance dessert incontestablement la santé psychologique

Les intervenants SAMU sont confrontés à de nombreuses interventions qui peuvent avoir un caractère traumatique (morts violentes, blessés graves, catastrophes, etc.). L'urgence, l'imprévisibilité et la gravité des pathologies font aussi partie de leur quotidien professionnel. Dans cette thèse, menée dans le Service d'Aide Médical Urgent de Paris, nous nous demanderons si cette activité professionnelle spécifique a des conséquences émotionnelles et psychologiques pouvant avoir une influence sur la santé psychique des intervenants SAMU.L'étude porte sur 69 intervenants SAMU : médecins, infirmiers, ambulanciers et étudiants en médecine. Notre dispositif de recherche comporte une étude qualitative (entretien clinique à visée de recherche) abordant le vécu psychologique des interventions médicales et une étude quantitative du stress professionnel (Inventaire de Stress Professionnel de Spielberger), de l'épuisement professionnel (Maslach Burnout Inventory), du stress de la vie quotidienne (Echelle de Stress Perçu de Cohen & al.), de la détresse psychologique (Questionnaire de Santé Générale de Goldberg), de l'état de stress aigu (l'Echelle Révisée d'Impact de l'Evénement de Weiss et Marmar) et de la pathologie post-traumatique (Questionnaire de Stress Post-Traumatique de Watson & al.). Afin de mettre en évidence la spécificité des situations de stress et des réactions psychologiques du personnel SAMU, nous avons également constitué un groupe contrôle de 51 sujets provenant de trois services différents de celui du SAMU (hépato-gastro, endocrinologie et chirurgie digestive). Contrairement à nos attentes, cette étude montre que les intervenants SAMU présentent des niveaux faibles de stress professionnel, d'épuisement professionnel, de stress aigu et de détresse psychologique par rapport aux personnels des services d'hépato-gastro, d'endocrinologie et de chirurgie digestive. Sur l'ensemble de la population SAMU, nous n'observons qu'un seul état de stress post-traumatique. Il semble que ce ne soit pas les interventions les plus exceptionnelles qui soient les plus stressantes et les situations de stress professionnel, pourtant fréquentes chez les intervenants SAMU, sont davantage perçues dans une dimension stimulante. Au regard de ces résultats, nous nous sommes interrogés sur les capacités des intervenants à faire face aux situations professionnelles difficiles. La mise en évidence de stratégies défensives, lors des entretiens cliniques, nous éclaire. Ces dernières sont nombreuses, elles sont portées collectivement et semblent être indispensables pour mieux répondre aux situations de stress

Purpose: This study aims at comparing professional rescuers and ordinary people, the way they react and they adjust to stress on different levels (perceived stress, neuroticism intensity, coping strategies, locus of control and posttraumatic stress disorder severity level ) in order to highlight the differences, the similarities and the links between the variables. The following hypothesis has been expressed: both groups (rescuers and job councellors) are different regarding perceived stress, neuroticism intensity, locus of control, the type of coping and the postttraumatic stress disorder severity level. Method: Population: 42 male individuals aged 24 to 55 (survey group: sea and moutain firefighters, High Mountain Squad gendarmes, bomb disposal experts, helicopter pilots and mechanics) and 42 individuals (33 women and 9 men) aged 24 to 59 (control group: job councellors) Tools: 5 scales have been filled in : PSS (perceived stress), EPI (neuroticism - extraversion / introversion), the Levenson scale (locus of control), WCC (coping) and QSPT (posttraumatic stress disorder). Results: the results bring to light that the rescuers show a lower level of perceived stress, of neuroticism, of external (P and C) and internal LOC, of emotional coping, of social support searching and of PTSD severity level than the job councellors. However, both groups do not differ significantly regarding the problem-focused coping. Conclusion: the harmful influence of environmental factors on the personality seems to determine the adjustment to stress and its traumatic consequences

The Central African crisis has led to the forced enrolment of children and so it has oriented me towards the following research : a psycho - analytical and psycho - pathological approach of Central African and Chadian soldier children ' s identity issues particularly once they are disarmed . These children have been through the DDR ( Disarmament , Demobilization and Reinclusion ) programme in Chad . I have been able to start such a research thanks to a training period at the UNICEF and with their team in N' Djamena . This research is based upon precise observations , interviews and therapeutic activities conducted by resorting to drawings , oral exchanges , all of which have served as clinical materials and have enabled me to show that a soldier child , once disarmed , continues his identification process with his leader even when placed at the Transit and Orientation Centre ( TOC ) . This very process reveals itself through the child acting out at the Centre. How is it possible to really understand a soldier child ' s attachment to armed groups and forces ? How can we usefully accompany him so that he can finally and definitely escape this terrible hold on him ? Thus , in order to better identify these key questions , we have studied the soldier ' s child behaviour in his social , cultural , and economic environment Moreover , we have also analysed the period when he was being helped by the DDR programme in N' Djamena ( Chad ) . So , the results demonstrate that these children continue to idealise their leaders because the DDR programme has flaws that partly prevent them from reintegrating and it eventually leads them to leave the Centre in order to join again their first membership group . This research on former Chadian soldier children has proved that the lack of psychological support will then increase their trauma even after several years . In this case , a psychological and educational support must be implemented both for a better identity self - construction of the person / child and to restart the process of building up an emotional psychology of the subject and also to help him become more mature . Furthermore , to enrich this theory as well as the clinical practice , this research has led to an analysis of the various strategies that have been set up to identify the difficulties but also to suggest concrete solutions for the parents and the different communities so that they can all be sensitized about the terrible and devastating consequences of armed wars and conflicts on these children
L'entité limite : entre la clinique séméiologique et la clinique projective. Étude sur une population tunisienne by Salha Saida( )

This study concerns two samples of Tunisian patients: a clinical group (N = 41) with subjects who received a diagnosis of Borderline personality disorder in reference to DSM-IV (APA, 2003) by their psychiatrist and a control group of schizophrenic patients (N = 27). We used the Rorschach comprehensive system (Exner, 1993) in order to identify, as a first purpose, the characteristics of the Rorschach's borderline functioning based on the Kernberg's description of the borderline organisation (1997). In parallel, we aimed, to verify whether the Rorschach's functioning characteristics are able to distinguish our clinical group from the Schizophrenic group. Furthermore, both of groups were tested once in the beginning of our study and again, with an interval of 6 at 9 months in order to study the stability of the Rorschach's variables. The results indicated that seven (7) variables distinguish our group of Borderline subjects from the schizophrenics. These variables represent highly relevant indicators of borderline functioning. Nevertheless, our clinical group is heterogeneous. In fact, different ranges of functioning have emerged. Elsewhere, there is no significant difference between the most variables in T1 and T2 but the stability may differ depending on the underlying functioning
