The purpose of this silent presentation is to demonstrate methods of eliciting the bulge sign, indicative of a small effusion of the knee. This objective is achieved in four step-by-step demonstrations on clinical subjects. In this presentation small effusions of the knee are demonstrated by a small bulging area noted on the side of the knee. Methods for detecting these effusions include compressing the medial aspect, followed by stroking the lateral aspect, and then stroking the medial aspect and tapping the lateral aspect of the knee. In the third method, the suprapatellar pouch is compressed, and the lateral aspect is stroked producing the bulge. Finally, in the fourth method, the lateral aspect is compressedand the medial aspect is stroked, and the bulge is again noted. Each method is demonstrated several times in succession
