This data serves as the basis for the growing use of real world evidence in medical decision-making. However, the data itself is not evidence. Analytical methods must be used to turn real world data into valid and meaningful evidence. Introduction and Objectives: Hepatitis C virus (HCV) infection is a leading cause of chronic liver disease with life threatening sequelae such as end-stage liver cirrhosis and liver cancer. It is estimated that the infection annually causes about 86,000 deaths, 1.2 million disability adjusted life years (DALYs), and ơ of the liver transplants in the WHO European region [1]. Presently, only antiviral drugs can prevent the progression to severe liver disease. Pegylated interferons combined with ribavirin are considered as current state-of-the-art treatment. Objective of this investigation was to assess the market uptake of these drugs across Europe in order to find out whether there is unequal access to optimised therapy.

Abstract: Ziele <br>Seit 1971 erfolgt in Deutschland die jährliche, opportunistische Früherkennungsuntersuchung des Zervixkarzinoms. Durch die Etablierung dieser S3-Leitlinie wird zum einen eine wichtige Forderung des Nationalen Krebsplans zum Zervixkarzinom-Screening erfüllt. Zum anderen kann die S3-Leitlinie wesentliche Informationen und Hilfestellungen für das geplante organisierte Zervixkarzinomscreening in Deutschland geben.<br><br>Methoden <br>Mit finanzieller Unterstützung durch die Deutsche Krebshilfe wurden durch 21 Fachgesellschaften evidenzbasierte Statements und Empfehlungen (GRADE-System) zu Screening, Management und Behandlung von Zervixkarzinom-Vorstufen erarbeitet. Zwei unabhängige wissenschaftliche Institute haben systematische Reviews für diese Leitlinie erarbeitet.<br><br>Empfehlungen <br>Der zweite Teil dieser Kurzzusammenfassung behandelt u. a. Abklärung, Therapie und Nachbetreuung zervikaler Dysplasien. Im Hinblick auf Nichtteilnehmerinnen am Screening empfiehlt die Leitliniengruppe erneute Einladungsschreiben oder eine HPV-Selbstabnahme. Ab einer Zytologie von Pap II-p in Kombination mit einem positiven HPV-Befund sollte eine Kolposkopie zur weiteren Abklärung durchgeführt werden, ebenso bei einem positiven HPV 16 oder HPV 18 Screening Test. Ein alleiniger auffälliger Pap-Abstrich sollte eine Triage mittels HPV-Test oder p16/Ki67 Dual-stain zur Folge haben

Background: Personalized medicine has the potential to allow patients to receive drugs specific to their individual disease, and to increase the efficiency of the healthcare system. There is currently no comprehensive overview of personalized medicine, and this research aims to provide an overview of the concept and definition of personalized medicine in nine European countries. Methods: A targeted literature review of selected health databases and grey literature was conducted to collate information regarding the definition, process, use, funding, impact and challenges associated with personalized medicine. In-depth qualitative interviews were carried out with experts with health technology assessment, clinical provisioning, payer, academic, economic and industry experience, and with patient organizations. Results: We identified a wide range of definitions of personalized medicine, with most studies referring to the use of diagnostics and individual biological information such as genetics and biomarkers. Few studies mentioned patients' needs, beliefs, behaviour, values, wishes, utilities, environment and circumstances, and there was little evidence in the literature for formal incorporation of patient preferences into the evaluation of new medicines. Most interviewees described approaches to stratification and segmentation of patients based on genetic markers or diagnostics, and few mentioned health-related quality of life. Conclusions: The published literature on personalized medicine is predominantly focused on patient stratification according to individual biological information. Although these approaches are important, incorporation of environmental factors and patients' preferences in decision making is also needed. In future, personalized medicine should move from treating diseases to managing patients, taking into account all individual factors

Ähnliche wie in früheren Reviews konnte auch die Metaanalyse durch Kleijnen Systematic Reviews einen besseren Schutz vor einem invasiven Zervixkarzinom durch ein HPV-basiertes Screening im Vergleich zur Zytologie zeigen. Daher empfiehlt die Leitliniengruppe - entsprechend den Richtlinien des Gemeinsamen Bundesauschusses (G-BA) - ein HPV-basiertes Screening mit Intervallen von mind. 3 Jahren für Frauen ab 35 Jahren. Ein Co-Testing kann ebenfalls durchgeführt werden. Frauen zwischen 20 und 35 sollten ein zytologisches Screening alle 2 Jahre erhalten
