In France, between 1981 and 1985, around one thousand children have been exposed potentially contaminated human growth hormone which can transmit the Creutfeldt-Jakob disease (CID). To date, 55 children have developed CJD. The objective of this work was to evaluate the risk of iatrogenic CID in children treated by hGH. The first step of this work was to identify more precisely the calendar period during which contaminated hGH had been distributed. Several statistical analysis have shown that the relative risk of iatrogenic CID was maximum in 1984. Several problems surround the methods used for this analysis. Hence, it was not possible to provide a quantitative estimate of the risk. In the second step of this work we developed a back-calculation model to estimate the incubation period of the disease. The estimated median incubation time ranged between 8.5 and 9.5 years depending on the statistical distribution of the incubation period assumed. Among the 55 possible and probable cases of iatrogenic CJD, 49 were homozygous for the codon 129 Prion Normal Protein (PRNP) polymorphism. Our estimate of the incubation period is valid only for homozygous. In heterozygous subjects, the data and the model suggests a longer incubation period but it was not possible to estimate it. In a second part of the work, we have developed and validated by a simulation study, a Bayesian approach designed to identify the precise contaminated batches of growth hormone. Analysis based on this method showed that between 1984 and 1985, 7 of 13 hGH batches were likely to be contaminated by the CJD agent. Our results suggest that the number of cases due to human growth hormone exposure should continue to rise during several years. The follow-up of this cohort is necessary in order to estimated the incubation period in heterozygous subjects
