It has been demonstrated that infliximab is an efficient treatment of Crohn's disease. However, whether and when to stop the infliximab treatment remains an important unanswered question. The aims of our study were to determine the time-to relapse post infliximab withdrawal, in patients who had obtained a stable infliximab-induced corticosteroid-free remission and to examine factors associated with this relapse. This retrospective single centre cohort study examined patients from the Henri Mondor Hospital who were treated with infliximab between April 2000 and July 2008. Thirty eight patients received a "short bridge" therapy, that is to say that they received only 3 infliximab infusions (at weeks 0, 2 and 6). Twenty-three patients received a "long bridge" therapy, that is to say that they received 3 infliximab infusions (at weeks 0, 2 and 6) followed by scheduled infliximab therapy every 8 weeks for at least 6 months; infliximab was stopped in patients who had obtained a stable corticosteroid-free remission. The median follow-up time was 95 months (IQR=53-105) and 59 months (IQR=36-74) respectively. The median time-to relapse after infliximab withdrawal was 25 months (IQR=6-73) and 20 months (IQR=6- ) respectively. In univariate analysis, the female sex (p<0.001), active smoking (p=0.001) and a CRP level at the first infliximab infusion inferior or equal to 5mg/L (p=0.05) were associated with a higher risk of relapse. In multivariate analysis, 2 factors were associated with a higher risk of relapse, the female sex (HR=2.38 [IC95%=1.14-4.94], p=0.02) and active smoking (HR=2.98 [IC 95%= 1.40-5.96], p=0.004). Retreatment with infliximab after relapse was effective and well tolerated in the vast majority of patients. These results needs to be further investigated via prospective controlled randomised studies
