The purpose of the present article is to evaluate the functional status of persons who suffered from spinal cord injury more than 20 years ago. It presents the most common age-related medical complications which worsen the functional status of the patients and thus affects their chronic disability

Background. Some years after the injury paraplegics begin to suffer from painsin the area of shoulder joints, pains most often resulting from the degenerative changes in the rotator cuff. The goal of the investigation is to show how often such changes appear and whaf they mean to a person whose locomotor abilities are already seriously impaired. Methods. Between 1997 and 7999 65 paraplegics who had sustained a spinal cord injury at least five yearsbefore were treated for acute or chronic shoulder pains. No paraplegics actively engaged in sports were included in the group. All of the patients were aged more than 50 years. In all of them the diagnosis was established by means of an ultrasound examination of the painful shoulder joint, preceded in each of them by the xray diagnostics of the shoulder joint. All were subject to out-patient treatment. Results. In 85% of the cases as treated the ultrasound diagnostics revealed degenerative changes of the rotafor cuff, in 12% signs of tendninitis of the m, supraspinatus, and in 3% only a partial rupture of the rotator cuff. No complete rupture of the rotator cuff was diagnosed. Most of the patients were treated by physical methods. Pain-relieving infiltrations were applied in the acute cases only. Combined treatrment was decided upon where physical therapy produced poor pain-relieving results. Conclusions. Degenerative changes of the rotator cuff most often result from a changed way of life involving permanent dependence ofthe patient on the use of the wheelchair, whereat his shoulder joints are overworked. As shoulder pains add further impairment to the paraplegic state, it is imperative to take proper preventive measures an important aspect of which is the choice of suitable orthopaedic aids
